Traditionally West Virginia’s frail and chronically disabled have been confined to nursing homes or other institutional care facilities. Now there are in-home alternative approaches to care. The Community Care Program and Medicaid Waiver Program are in-home care programs that provide personal assistance to eligible adults. These services are medically oriented activities or tasks to be implemented according to a Nursing Plan of Care that is developed and supervised by a registered nurse.  These services enable people to meet their physical needs.  As a Homemaker Agency, KVSS provides eligible clients with trained homemakers or caregivers that perform health care services, assistance with personal hygiene, nutritional support and environmental support.

 

Services Provided:

·     Prescribed by a Physician

·     Necessary to long term care of the recipient’s health and safety

·     Provided within the recipient’s place of residence

·     Provided pursuant to a “Plan of Care” and periodically monitored by a KVSS Registered Nurse

·     Rendered by an individual who has met the basic training requirements. Family member (other than the spouse or parent of a minor child) may be eligible to provide care.

 

VA Homemaker Services:

Veterans may be referred for services from the Veteran’s Administration, Clinics, Community Agencies or a family member.  The goal is to enable veterans to remain in their own homes.

Services Provided:

·     Bathing & Dressing

·     Meal Preparation & Light Housekeeping

·     Laundry & Shopping

 

Creative Care Program - Private Pay:

This program is designed to provide services to those individuals who wish to hire a Care Provider to assist with a variety of tasks.

Services Provided: 

·     In-Home Care (Bathing, Grooming, Meal Preparation & Environmental Support).

·     Medical Appointments – accompanying a client to appointments and making a brief report to the family.

·     Shopping and errands.

·     Hospital – sitting with the client to alleviate fears and provide liaison to family if required.

·     Long-Term Care Facility – brief or extended visits with client in facility and making a brief report to the family.
